These will be arranged by the Department of Health.<\/p>\n\n\n\n<p>Others will be given by a doctor or health professional and are your responsibility as a parent or carer to arrange.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"school-and-vaccination\">School and vaccination<\/h2>\n\n\n\n<p>When you fill out your child\u2019s school enrolment form, you will be asked whether your child has received particular vaccinations.<\/p>\n\n\n\n<p>You may be asked to provide evidence.&nbsp; This is usually in your child\u2019s health record, commonly known as the \u2018blue book\u2019 or you can provide a letter from a doctor or local council.<\/p>\n\n\n\n<p>Immunisation records can be accessed through your&nbsp;<a href=\"https:\/\/www.humanservices.gov.au\/individuals\/services\/medicare\/medicare-online-accounts\" target=\"_blank\" rel=\"noreferrer noopener\">Medicare online account<\/a>,&nbsp;through&nbsp;<a href=\"https:\/\/my.gov.au\/LoginServices\/main\/login?execution=e1s1\" target=\"_blank\" rel=\"noreferrer noopener\">myGov<\/a>&nbsp;or by using the Express Plus Medicare mobile phone app.<\/p>\n\n\n\n<p>The Australian Immunisation Register \u2013 Whole of Life Register (AIR) also keeps a record of immunisations given to children under the age of seven years.<\/p>\n\n\n\n<p>Statements of your child\u2019s vaccination history can be requested from AIR by phoning <a href=\"tel:1800 653 809\" target=\"_blank\" rel=\"noreferrer noopener\">1800 653 809<\/a> or go to the&nbsp;<a href=\"https:\/\/www.health.gov.au\/topics\/immunisation\/getting-vaccinated\/check-immunisation-history?language=und#the-australian-immunisation-register-air\" target=\"_blank\" rel=\"noreferrer noopener\">Check your child\u2019s vaccination status&nbsp;web page<\/a>.<\/p>\n\n\n\n<p>If proof is not available, a statutory declaration form can be obtained from your school or from the Department of Health.<\/p>\n\n\n\n<p>The dates for vaccination sessions at your child\u2019s school will be communicated to parents and carers. You will receive a consent form for your child to have the vaccination. It is very important that you read and fill out this form correctly and return it by the due date.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"what-if-there-is-an-outbreak-of-infectious-disease\">What if there is an outbreak of infectious disease?<\/h2>\n\n\n\n<p>If your child is not immunised, you may be asked to keep your child at home if there is an outbreak of disease that is infectious, contagious or harmful to others.<\/p>\n\n\n\n<p>If your child is immunised, your school will give you advice from the public health authority about whether attending school during an outbreak is safe.<\/p>\n\n\n\n<p>Always follow the advice from your school as it is based on expert medical advice from the Department of Health.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"infectious-diseases\">Infectious diseases<\/h2>\n\n\n\n<p>You must contact the Department of Health if your child ever contracts one of the following infectious diseases:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Measles<\/li>\n\n\n\n<li>Meningococcal disease<\/li>\n\n\n\n<li>Mumps<\/li>\n\n\n\n<li>Whooping Cough<\/li>\n\n\n\n<li>German Measles<\/li>\n\n\n\n<li>Tuberculosis.<\/li>\n<\/ul>\n\n\n\n<p>The Department of Health can be contacted on&nbsp;<a href=\"tel:1800 671 738\">1800 671 738<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"where-to-go-for-more-information\">Where to go for more information:<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>your school or child care facility<\/li>\n\n\n\n<li>your local council<\/li>\n\n\n\n<li>your family doctor or practice nurse<\/li>\n\n\n\n<li>the Department of Health on&nbsp;<a href=\"tel:1800 671 738\">1800 671 738<\/a><\/li>\n\n\n\n<li>visit the&nbsp;<a href=\"https:\/\/www.health.gov.au\/health-topics\/immunisation\/immunisation-throughout-life\/national-immunisation-program-schedule\" target=\"_blank\" rel=\"noreferrer noopener\">National Immunisation Program Schedule<\/a><\/li>\n\n\n\n<li>find more information on <a href=\"https:\/\/decyp.dev.ionata.com\/ja\/wellbeing\/health-and-physical-care\/infectious-diseases\/\">infectious diseases<\/a>.<\/li>\n<\/ul>","protected":false},"excerpt":{"rendered":"<p>Some vaccinations are provided to your child on school premises by a doctor or health professional.
