It is given based on what the school has been doing to support the student in the last 12 months.&nbsp;&nbsp;<\/p>\n\n\n\n<p>The educational adjustments made for students need to be written in the student\u2019s Learning Plan.&nbsp;&nbsp;<\/p>\n\n\n\n<p>The student\u2019s learning plan is developed in conversation with the student and family. A learning plan sets the goals the student will be working on each year and the progress a student makes.&nbsp;<\/p>\n\n\n\n<p>A Learning Plan tells us about the educational adjustments that are being made for each student with a disability to meet their goals.
